Supported accommodation/Outreach Support for mental health
If you or someone you know who is living with a mental health condition and requires personalised support to live independently, we offer specifically designed supported accommodation or outreach support that we can deliver in individuals’ homes.
We provide self-contained one-bedroom flats offering a unique service to suit your needs. Therefore, whether you are living with bi-polar disorder, depression, psychosis, and other complex needs, we can help.
Our supported accommodation and outreach services are focused on achieving positive outcomes for individuals by working with them to develop and maintain life skills such as:
- Food Shopping
- Cooking
- Support with Finances
- Support to maintain tenancies
- Support to lead healthy lifestyles
- Social life and leisure activities
- Voluntary or Work placements.
This type of support focuses on helping you build on your strengths, providing you with opportunities to engage with community-based services and resources.
The tenancy comes with four hours of support a week, for up to two years.
In our homes, you can expect good quality support with help from a team of support worker who will help in a variety of ways so you can develop daily living skills. Support workers can provide a set amount of hours of support per week, which can be tailored to your specific needs or circumstances. This can also consist of one-to-one support from a peer mentor, getting involved in specialist group workshops and group activities, and more.
The accommodation is a stepping stone to independent living.
